Reason for request. Please note, you will not be charged for your first uniform or replacement of items that you have had for over a year

Employees receive up to one free uniform each year. Upon termination employee must return their uniform or they will be charged $90 out of their final pay. If ordering extra or replacement items that you have had for less than 1 year, there will be a payroll deduction in the amount listed for each item ordered.
